A new US Coast Guard report reveals that OceanGate CEO Stockton Rush "completely ignored" warning signs and critical data leading to the Titan submersible's implosion in June 2023.
The 335-page report found OceanGate had "critically flawed" safety practices and a toxic workplace culture, saying that Rush’s “negligence” contributed to the deaths of the five people on board.
The Coast Guard identified evidence of a potential criminal offence, saying they would have recommended a separate criminal investigation by the Department of Justice had Rush survived.
The report said that a lack of third-party oversight and experienced staff allowed Rush to bypass vital inspections and maintenance procedures.
It also noted discrepancies between OceanGate's written safety protocols and actual practises, alongside a failure to investigate a 2018 whistleblower complaint.
